Confirmation Report - Indicate when no location is assigned to an occurrence

Currently the confirmation report shows a table for assigned locations and their instructions on every occurrence, but does not display anything at all if no location is assigned.
Instead, we should show the table and include the text "No location reserved" in the location name field.
